As part of receiving the Scholarship each Scholarship recipient must commit to work with mothers giving birth in rural Tennessee hospitals or mothers living in a rural Tennessee County where there are no birthing hospitals (in other words, mothers in rural Tennessee Counties) for two years after becoming a CBS. If you would be willing to provide lactation support for mothers giving birth in a rural Tennessee hospital or mothers living in a rural Tennessee County where there are no birthing hospitals (mothers in rural Tennessee Counties) for two years after becoming a CBS, please complete and sign this Application. The following Counties are NOT rural Tennessee Counties: Davidson, Hamilton, Knox, Madison, Shelby and Sullivan.
